I am looking for information/compatibility for a patient who will probably start using Dupixent (listed as L3 prob compatible) now that she is postpartum (stopped using in pregnancy) and has used topical steriods on nipples due to eczema. I do not know the corticosteriod used. Would pumping be recommended?

Because dupilumab is a large protein molecule with a molecular weight of about 147,000 Da, the amount in milk is likely to be very low. It is also likely to be partially destroyed in the infant's gastrointestinal tract and absorption by the infant is probably minimal. In regards to the steroids on the nipples, I would recommend avoiding to reduce the chances of thinning tissue and direct infant exposure. If she is doing this regularly, please call us with the name and dosage being used so that we can provide more insight regarding risk vs. benefit.
