    Pumping and discarding for 4 days after receiving methotrexate is our recommendation. Toddlers get much less milk per day in relation to their body weight as compared with infants and they are better able to tolerate incidental medication exposure. Given that you waited 17 days, it is highly unlikely that your baby will experience any complications.

      I am writing from Croatia, Europe, and I am seeking guidance regarding breastfeeding after receiving methotrexate for an ectopic pregnancy.
      I received a single 100 mg intramuscular (IM) dose of methotrexate on June 9, 2026, as treatment for an ectopic pregnancy.

      I had been breastfeeding my 20-month-old child until I received the methotrexate injection. Following medical advice, I have currently paused breastfeeding and am seeking guidance on when, if at all, it would be safe to resume.

      The physicians caring for me advised that breastfeeding must be stopped completely and that I should not breastfeed again for the next six months. However, I am not confident that this recommendation is based on current evidence regarding lactation and methotrexate exposure.

      As I have received conflicting information locally, I am hoping to obtain an evidence-based recommendation from specialists with expertise in medications and breastfeeding.

      Could you please advise whether there are established recommendations for breastfeeding after a single-dose intramuscular methotrexate treatment? Specifically, I would like to know:

      - How long breastfeeding should be withheld before it is safe to resume (after a single 100 mg IM dose of methotrexate)?

      - Whether there are any special considerations for a toddler who is 20 months old and not exclusively breastfed.

      I would be very grateful for any information, references, or guidance you can provide, as there appears to be limited expertise on this specific issue where I live.
