  • Zepbound and breastfeeding

I have read much in the forum so far and feel okay starting zepbound while still breastfeeding but I figured I would ask myself for reassurance with this decision.

I’m overweight and plan to start zepbound 2.5mg/month. I’m currently nursing my 19 month old 1-2 times a day. She eats a varied diet, drinks plenty of water, and we offer her whole cow’s milk at breakfast and dinner. She has self-weaned down to this point and I’d like to let her continue to do so. I don’t think she will nurse for very much longer and it seems unlikely she is relying on my milk for much other than bonding and comfort.

Should I consider it safe to start zepbound and still allow her to nurse?

thank you so much for the work you do.

Thank you for your thoughtful message and for taking the time to research this carefully. Based on what you've shared, it is reassuring that your 19-month-old is primarily relying on solid foods, water, and cow’s milk for nutrition and is only nursing occasionally for comfort. At this stage, the nutritional contribution of breast milk is typically minimal, which reduces the potential impact of any medication transfer through milk.

Currently, there is no direct clinical data available on the use of Zepbound during breastfeeding. However, given its large molecular size and the way similar medications behave, only very small amounts, if any, would be expected to transfer into breast milk. Additionally, because your child is older and breastfeeding infrequently, any potential exposure would likely be extremely low and not expected to cause harm.

Of course, because long-term studies are lacking, it’s important to monitor for any unexpected changes, such as poor feeding, gastrointestinal upset, or unusual behavior, although these risks are considered theoretical and low. Ultimately, it sounds like you are making an informed decision that balances both your needs and your child’s well-being, which is exactly what we hope to support.

KJC, I'm looking to start it as well. My little is 19m and only nurses for comfort and is done quickly.

if you've started, have you noticed your little having any reactions?
