Hello, I am 6 months postpartum, exclusively breastfeeding a very healthy baby who is above average height and weight. I already gained some weight before pregnancy during IVF process, and then during pregnancy, and post partum I gained even more because my Hashimato Thyroid got out of balance and I did not realize it. I am super depressed about it and I am seriously considering starting Mounjaro. (Also because I want a second pregnancy once my baby turns around 1-1,5 years and I absolutely cannot get pregnant with this extra weight). My milk supply has been very very good from the beginning, my baby is eating well. My research shows that tirzepatide does not enter breast milk but the concern is more about the decrease in milk supply through weight loss. If I were to continue to eat healthy food and drink lots of water, is there still a risk for that? I hope to breastfeed at least until baby turns 1 year old but I also cannot continue like this. Looking for some advice.

One of the concerns we have in using the GLP-1 medications in breastfeeding an infant before 12 months is that it may decrease the nutritional content and fat content of milk. In infants that are depending on breast milk for their nutrition, this could be problematic. So, while we have not seen the medication actually get into the milk, it could have a negative impact. We worry a lot less about the potential impact in infants over 12 months. We wrote an article about weight loss during this time and have a calculator that we developed to help you manage your caloric intake while maintaining milk supply. You might find it helpful.
