    A mother (with a 3-month-old baby) receives Opalmon or Limaprost alfadex (5 mcg) 1 x 3 per day. I am not able to find this medication in Hale's Medications & Mothers' Milk 2023. Please suggest. Thank you very much.

    Hi LC:

    The molecular weight (380) and moderate plasma protein binding (about 88%) suggest that limaprost will be excreted into breast milk. With TID dosing, it will be difficult to avoid drug-in-milk as the half-life is 1.6 hours. There are similar medications that are applied topically (Bimatoprost) that would be preferred--there isn't any additional data, but the amount of drug systemically available to transfer into the milk will be MUCH lower.
