    hey guys!!
    i have lupus and with that I have plaque psoriasis. For 6 months I’ve been prescribed otezla and I have yet to try because I don’t know anyone who’s actually taken this to say how safe it is. My daughter is still breastfed some and she’s 2 years old! please help??

    Apremilast is a phosphodiesterase-4 inhibitor used in the treatment of adults with active psoriatic arthritis. It is currently unknown if it is secreted in human milk. Due to its high pKa, low molecular weight, and good oral absorption, care should be exercised when apremilast is given to a nursing woman. Its chemistry, low molecular weight, high pKa suggests it could enter milk.

    I doubt at 2 years that the dose your infant would receive from milk is high enough to cause any problems.

    The drug peaks in your plasma at 2.5 hours, so I'd wait 4-6 hours before breastfeeding after taking it. Observe infant for: Diarrhea, Nausea, and Vomiting.
