  • Clevidipine (Cleviprex)

I have a day 4 postpartum ICU patient with cardiomyopathy and pulmonary edema who is receiving IV Clevidipine. I have not found it in the most recent edition of Medications & Mothers' Milk nor on lactmed, and of course all I can find online is that we have no date for breastfeeding. Her milk is now in, great supply, pumping 2-4 oz at a time. Baby is not latching currently as she has a severe tongue tie and latching is too painful for mama. Here's what I could find about pharmacokinetics:
    Pharmacokinetics

    Onset: 2-4 minutes

    Protein Bound: 99.5%

Metabolized in blood and extravascular tissues

    Half-Life: Initial 1 minute; terminal 15 minutes

    Excretion: Urine (63-74%); feces (7-22%)

......opinions? Seems to me it should be pretty safe in terms of excretion into milk.

She would need to wait 1 hour after the drip has stopped. It will probably not transfer to milk to a large degree. Protein binding is >99.5%. Terminal half life is 15 minutes.
