Just wondering what my safest options of anit-depressant/anxiety medications are while breastfeeding my 6 week old... Also is the supplement SAM-e safe while breastfeeding? Thanks in advance!

Dear trishb01,
Sertraline (Zoloft) is the preferred antidepressant during breastfeeding because studies show only 0.4 % to 2.2 % of the dose gets into the breastmilk. The time sertraline is most concentrated in breastmilk would be 7 to 8 hours after the dose so avoid breastfeeding at that time to decrease the amount of drug the baby is exposed to. The typical dose range for adults is from 50 to 200 mg daily. Observe for diarrhea and tremor although not likely since the amount of drug is low. Lorazepam (Ativan) is the preferred benzodiazepine during breastfeeding because studies show that only 2.9 % of the dose gets into the breastmilk. The time lorazepam is most concentrated in breastmilk is 2 hours after the dose. The typical adult dose is up to 1-3 mg two to three times daily. Observe for sedation. It is important for your depression to be treated as untreated depression itself can have untoward effects on your infant. S-Adenosylmethionine (Sam-e) is a naturally occurring substance in most body tissues. The drug appears to increase the central nervous system processing of dopamine and serotonin which are chemicals in the brain that regulate many body functions. One study showed improvement in depression by 7 days and further improvement by 14 days with no serous side effects in the (non-breastfeeding) patients. There are no reports of Sam-e use in breastfeeding women.
