  • Doxycycline and Toddler Nursing

    Pt receiving doxycycline 200mg IV BID for Lyme treatment (anaphylaxis with Rocephin which is typically used for Lyme.) Toddler is almost 3, nurses in once in the morning and once at night for a few minutes and is easily redirected. Can pt continue these brief sessions, as little milk is being consumed?

    It depends on how long she is going to continue this treatment. We all know that tetracyclines can produce dental staining. Mothers with newborns < 1 year of age will have little risk of increasing dental staining as the enamel layer of the teeth is not underdevelopment until about 12 months.

    Those with older infants may have problems with dental staining, and it depends on how long the exposure lasts.

    I generally suggest no more than 3 weeks of continued exposure from oral or IV administration. In this situation with an infrequent feeder, the risk is probably remote. But if this mom is being treated for 6 months or something longer, I'd get a little uncomfortable suggesting breastfeeding.

    Three weeks OK, more is more risky.
