  • Ciprofloxacin while breastfeeding- following 2 previous antibiotics

    I am exclusively breastfeeding my 7 week old daughter and was prescribed Ciprofloxacin 250 mg BID for 3 days today for a UTI that has not resolved after two previous antibiotics (Augmentin & cefuroxime). I have received two different versions of guidance. My OB and the pediatrician have both said official guidance from the FDA and Up to Date state not to breastfeed while on Cipro and for 2 days after completing the medication. However, I’ve read and been told by the pharmacy that I can breastfeed but to wait 4 hrs after a dose. Is it safe to continue exclusively nursing my baby on Ciprofloxacin? Also, is it safe that this will be the third antibiotic she’s been exposed to in the last 3 weeks? Augmentin 4/19-4/23 and cefuroxime 4/25-5/02.

    Ciprofloxacin passes into breast milk in small amounts and is generally safe to use while breastfeeding, especially for a short course like 3 days and in a healthy, full-term baby. While antibiotics can temporarily affect a baby’s gut, breast milk helps protect and restore their microbiome. Even with recent exposure to other antibiotics like Augmentin and cefuroxime, the benefits of continuing to breastfeed outweigh the risks. At most, your baby might have mild side effects like diaper rash or a bit of diarrhea, but these are usually short-term. As long as your baby isn't at high risk for GI issues (premie, history of severe diarrhea, etc), I wouldn't worry about it too much.
