    I have a patient who will be delivering her baby next week and has been taking 40mg of oxycontin bid and 6 extended release oxycodone 15 daily. She is dealing with a chronic pain problem but would like to breastfeed. How can I advise her?

    If the infant has been exposed to this during pregnancy, then the risk to the infant after delivery is lessened, and the amount in milk may actually reduce withdrawal symptoms.

    Thereafter, the infant will have to be closely monitored for side effects of opiates, which include respiratory depression, constipation, etc.

    Most importantly, if the mom uses the same dose every day, then the risk will develop tolerance and not be at high risk. IF the mom varies her dose, high one day and low the next, then the infant is at greater risk.

    With these doses, the pediatrician should closely monitor the infant.
