I was wondering how safe is acupan for breastfeeding women? What is the bioavailability and half life? We are trying to get our anesthetists to stop prescribing Tramadol for post C/section analgesia and wondered if Acupan would be an acceptable alternative...cheers

Dear Chris,
Nefopam (Acupan) is a non-narcotic analgesic used for post-operative and musculoskeletal pain. The drug also works well for episiotomy pain. Nefopam is an alternative pain control method for patients who are opioid dependent as it is structurally related to diphenhydramine (Benadryl). The drug does not appear to cause respiratory depression. The RID (relative infant dose) is 2.6 % and is well absorbed orally. The half-life (T 1/2) is 3 to 8 hours. Nefopam appears to be compatible with breastfeeding. Alternatives that are also compatible with breastfeeding are morphine with RID 9.1 % but is poorly orally absorbed (26%), & T 1/2, 1.5 to 2 hours; oxycodone RID 3.5 % with 50% orally absorbed & T 1/2, 3 to 6 hours; and hydrocodone RID 3.1 to 3.7 % with complete oral absorption & T 1/2, 3.8 hours.
