    Today I received a question about methadone that I could not answer.

    The question "If a mother in a methadone program continues in the program while she is breastfeeding, she is on methadone maintenance. What happens to her baby when she weans? Does the baby have a risk of NAS?

    Dear Nikki,

    Abrupt cessation of breastfeeding during high dose Methadone therapy can result in neonatal abstinence. Infant should be weaned slowly. An example being if the mother is breastfeeding 8 times a day, drop to 7 times a day for about 5 days, then drop to 6 times a day for about 5 days, then to 5 times a day for about 5 days and trending down to nothing.
