  • Breastfeeding and Lorazepam, Quetiapine and Sertraline Combined Safety?

I am involved in the ongoing care on a breastfeeding dyad; exclusively breastfeeding. Baby is healthy, gaining weight well and baby is currently 20 weeks old.
There are maternal mental health issues and the dyad are currently admitted in a respite accommodation being monitored and looked after for mental health concerned.
The respite staff have prescribed the Mother Quetiapine, Sertraline and Lorazepam.
Quetiapine and Sertraline are both L2 on Hales database and Lorazepam is L3 but, from what I can understand, is considered safe although there is limited data. My query is around the safety with the combination of all three medications together with breastfeeding, can anyone advise?

The staff have advised the Mother not to breastfeed on Lorazepam therefore the Mother is not taking it, and she wishes to continue to breastfeed. She has asked for another opinion on these medications.

Hi there--I'd say it depends on the doses. You might call and talk through the case with our nurses at the InfantRisk Call Center in the morning. The phones open at 8am CST

My heart goes out to these mommas who sacrifice their health so their babies can continue getting their milk. We can almost always find a way to support them with safe treatments while breastfeeding.
