  • CGRP inhibitors/antagonists for migraine - Aimovig, Ajovy, Emgality

Hello! Now that the new migraine prevention drugs have been on the market for a little while (about a year in the case of Aimovig), I was wondering if there is any more advice, information, or experience about using these while breastfeeding (I am aware there are no data). My little is 2 years old and weighs 35lb. He nurses 2-3 times per day for 5-10 minutes. I have read the information on LactMed and understand the risks are likely low. I would be grateful for any information or recommendations. Thank you!

At this time, we do not have any data at any monoclonal antibody type of anti-migraine preparations. They are all large IgG monoclonal antibodies. Aimovig and Ajovy are IgG2 antibodies, which from experience, do not readily enter breastmilk. Emgality is an IgG4 antibody, which are believed to enter milk significantly.

I would not recommend Emgality. We have some data that suggest IgG4 molecules can enter milk at higher levels.
