    Dear Dr. Hale,
    Can you comment on the safety of Nevanac 1% opthamalic solution , Generic Name: nepafenac ophthalmic, dosing three times a day, one eye. The nursling weighs 32lbs and nurses 1-3 times a day about 4-5 days per week.

    Nurturing Family:

    Nevanac ophthalmic solution is a topical nonsteroidal agent (NSAID). Plasma levels reported are exceedingly low (Cmax=0.31 nanogram/ml) and milk levels will probably be 10-100 fold less than plasma...or essentially nothing.

    While we do not have specific data on this product in breastfeeding mothers, I would not be overly worried about its use. Thus far, none of the NSAIDs studied thus far produce signficant levels in milk that are clinically relevant. This one is even less likely due to the low initial starting dose used ophthalmically.
