• Golimumab / Simpni

    Hi

I am supporting a mother with a 2 week old infant. Mum has severe ulcerative colitis. She has been offered a new biologic therapy Golimumab but there is little information around this. I wondered if you had seen any recent information, or could shed light on the pharmokinetics to allow her and her doctors to make an informed decision regarding medication and breastfeeding.

    Kind regards

    Cscmadwife,

Golimumab is rated an L3-no data-probably compatible. Golimumab is an anti TNF antibody and acts similarly to other TNF inhibitors such as infliximab. There are no studies available on the transfer of the drug into human milk. Levels in monkey milk were 400 fold less than plasma levels. This suggests levels in human milk will be similarly low. Due to its size and structure, levels in milk will be low. Oral bioavailability will be low as well. Note: this product is commonly used with methotrexate, which can be hazardous to a breastfeeding infant. Monitor the infant for fever, constipation, weight gain, frequent infections.
