    Can you update us on both the issues r/t methergine in the immediate PP period, but also in use following discharge? I am particularly curious re the impact on milk supply that may occur from 3 days of use following a PPH at 1 wk. Mom pumped and dumped per her Peds advice. Thank you!

    We have an article posted on our site which addresses current research on methylergonovine (methergine) as it relates to breastfeeding safety. With respect to milk supply, we have found that a single post-partum dose will suppress prolactin levels, but doesn't seem to have much of a noticeable effect on supply. Existing studies are conflicted regarding this drug's effect on milk supply if used for more than a single post-partum dose. Some studies found no effect, others describe a very significant drop in milk production which is reversible on stopping the medication. No new research has been published on this topic in the last 20 years.

    Pumping-and-dumping is probably not necessary as very little of this medication makes it into the breastmilk.
