    I'm an IBCLC working with a mom and her 10 week old baby who has been gaining weight poorly despite mom's plentiful milk supply and baby's good milk transfer. I'm trying to figure out what's happening and am curious if the meds mom is on might be causing poor digestion or absorption in baby. Specifically these two meds:

    Apriso ER (Mesalamine) .375 g 2 tablets daily (AM)
    Lialda DR (Mesalamine) 1.2g g 4 tablets daily (2 AM, 2 PM)

    She is also taking prenatals, a calcium supplement, and fiber gummies.

    Baby is having very loose, watery, staining stools that mom reports as similar to diarrhea. She reports this is a new symptom within the past 3 or so weeks, however baby's weight gain has been poor since before the loose stools. I see that Mesalamine has caused diarrhea in some breastfeeding babies so I'm wondering if it could also be the cause of the poor gain. Thank you!

    You're correct, that in rare cases we've heard of diarrhea in breastfed infants. I'd suggest this mom switch to half breastfeeding and half formula. This should tell you pretty quickly if its the mesalamine and help with the infants weight gain.
