I have a client who just under went a total hysterectomy, her baby is 3 months old and her Doctor wants her on hormone replacement therapy. He has recommended Premarin. Mom is unsure about this medication. She is fully breastfeeding and doesn't want anything that will prohibit that. Any suggestions?

Dear Mommajuice,
Premarin contains estrogen which has the potential to decrease mom's milk supply. The decrease in supply varies depending on the individual and the dose of estrogen being taken. If her supply is well-established, she may be able to continue to successfully breastfeed her child. She needs to have a risk/benefit discussion with her physician about long term effects for herself if she chooses to wait to start hormone replacement until she weans the baby. The hormones levels present in the breastmilk are low and do not present an untoward risk to the breastfeeding infant.
