    Hi Dr Hale, your advice would be very appreciated.
    Thank you.

    Baby is 30 months old and breastfeeding frequently day and night. Child has nuphrotic syndrome but has only had it once in her life. Mum tests urine frequently to monitor. In remission for 9-12 months.

    Medications Mum has been prescribed:
    Dulexotine - Cymbalta, start on 30 mg then go to 60mg in one week.
    Quetipine Seroquel it's low dosage - 50mg or 100mg

    Mum does not want to wean if possible and wants to breastfeed in the safest way possible.

    Dear DRS10,

    Both Cymbalta and Seroquel are metabolized mainly by the liver. For patients with severe renal impairment, plasma concentrations of Seroquel were not different than patients who had normal renal function. For Cymbalta, the major circulating metabolites are not considered pharmacologically active. Since her child is older, the amount of milk consumed at this age is usually small also compared to an infant. Dr. Hale states that these medications are compatible with breastfeeding in this case. The mother and pediatrician should continue to monitor infant for signs of renal impairment.
