  • Clindamycin and baby with significant GI problems

    Hi-

I may have to take this to treat an absess on my torso (possibly MRSA, will know results in 2 days). Baby has allergic collitis and her GI specialist said I can not breastfeed for the week I am on the meds due to the risk to her GI system. I was just hoping I could get confirmation of this before we temporarily switch to formula for the week.

    Thanks so much.

    Dear dolifre,

Clindamycin is usually compatible with breastfeeding. The amount of your dose that enters breastmilk (RID) is 0.9 to 1.8 percent. The usual adult dose of clindamycin is 150 to 450 mg every 6 hours. The time the medication is highest in your milk is 45 to 60 minutes after your dose and the half-life is 2.4 hours (time it takes for half of the dose to be eliminated from your system). Because the amount of clindamycin in breastmilk is low, the risk of Clostridium difficle overgrowth typically is low, but because your infant has a predisposing GI condition the risk may be elevated. You should share the above information on the RID, half-life etc. with your infant's physician and have a risk/benefit discussion about withholding breastmilk while on treatment. Studies have shown that breastmilk protects the intestinal system of infants because of the immunities it provides. In fact, mother's of premature infants are requested to donate their breastmilk to their infants to prevent a serious intestinal infection called necrotizing enterocolitis. Hope this information helps.
