    Hi, is it safe to take Nifurtoïnol (Urfadyn PL) 6 months during breastfeeding, baby is 12 weeks old. thank you.

    Nitrofurantoin is generally considered safe to use in breastfeeding women, with exception in the neonatal period when the infant may have high bilirubin levels (first 3-4 weeks). Nitrofurantoin displaces bilirubin from its albumin binding site, thus increasing the "free" level in the plasma. In your case at 12 weeks, your infant would generally not be at risk.

    In a well-done study of 4 breastfeeding mothers who ingested 100 mg nitrofurantoin with a meal, the milk/serum ratio averaged 6.21 suggesting an active transfer into milk.[3] Regardless of an active transfer, the average milk concentration throughout the day (AUC) was only 1.3 mg/L. According to the authors, the estimated dose an infant would ingest was 0.2 mg/kg/day or 6.8% of the weight-adjusted maternal dose if they consumed 200 mg/d nitrofurantoin. The therapeutic dose administered to infants is 5-7 mg/kg/day.
