  • Breastfeeding while on suboxone and baby being tapered from morphine

Hi mabye someone can help us, my wife was on subutex throughout her pregnancy and immediately after she delivered her Dr. switched her to suboxone. We had met with a neonatologist very early in her pregnancy to discuss the side effects of her being on the medication thru her pregnancy. We asked him if she could breastfeed and he said she cannot because of the suboxone and her seroquel. The very same Dr now 9 months later says its ok for her to breastfeed. Unfortunately because of her being on sub her whole pregnancy our son has to be given morphine to ween him off the sub. She tried to breastfeed him and not too long after he started getting really bad withdrawl symptoms and he was doing really good up until that point. Im wondering if the blocker in the sub is being passed to him thru the milk and its reacting with the morphine he's taking. I know in adults if you still have an opiate in your system and take sub to early it sends you into full blown withdrawl. Is it possible that what is happening here? Thanks for your help!!

Suboxone is a combination of Nalaxone and Buprenorphine. Naloxone is an opioid antagonist, and it is poorly absorbed orally and plasma levels in adults are undetectable (<0.05ng/mL) two hours after oral doses. Its use in breastfeeding mothers would be unlikely to cause problems as its milk levels would likely be low and its oral absorption is minimal to nil.
