    Hi
    Pftizer recommends to not breastfeed for 6 hours after receiving Hemabate for PPH in their product monograph. Is it accurate that it can cause interference with closure of the PDA? What is your recommendation?

    Dear Meggie,

    No data are available on the transfer of carboprost into human milk. We do know that prostaglandins have brief half-lives and little distribution out of the plasma compartment. It is not likely carboprost will penetrate milk in clinically relevant amounts. Dr. Hale suggests that waiting four hours would eliminate most risk to the breastfed infant. Yes, prostaglandins are known to have an effect on the ductus arteriosus. Prostaglandin E1 dilates the ductus arteriosus. Some medications such as indomethacin inhibit prostaglandin synthesis and are given to close the PDA in premature infants.
