    Imuran together with mesalamine

    Hi,
    A pregnant mother with Crohn Disease asked me if she can breasfeed her child. She uses Imuran together with mesalamine witch both have warnings to use them with caution and have L3 classifications. She also uses Remicade but that is considered to be safe so I read in MMM.
    What can I advise this mother?

    Dear Erica,
    This mother should be advised that Imuran (Azathioprine) is an immunosuppresant agent and therefore the infant should be closely followed by his/her pediatrician for signs of immunosuppression. Although rare, mesalamine could cause watery diarrhea in the infant and therefore he or she should be closely monitored.
